Please prepare the following documents. Your account manager will request these after you submit the form.
ID: A State ID, driver’s license, or US passport.
Business Document: An EIN Letter or Articles of Incorporation (EIN is preferred).
Banking Document: A copy of the bank statement, with the full account number and name clearly visible. This can be a photo, image or PDF file downloaded directly from your bank’s website. Alternatively, you can provide a voided check or a bank account verification letter, signed by a bank representative.
